Abstract: Suicide among adolescents has shown a worrying increase in Brazil, especially among boys, and has become a public health problem. The literature indicates that the combination of biopsychosocial vulnerabilities, structural inequalities, exposure to violence, fragile protection networks, and difficulties in accessing qualified care favors the emergence of suicidal ideation. This study aims to analyze, through an integrative review, the scientific evidence published over the last ten years regarding the identification, management, and prevention of suicidal ideation among adolescents within the scope of Primary Health Care, with a focus on nursing practice. The search was conducted in the SciELO, LILACS, and Virtual Health Library (BVS) databases, using descriptors in Portuguese, English, and Spanish related to suicide, adolescence, mental health, and professional practices. After applying inclusion and exclusion criteria, 11 studies composed the final sample. The results indicate that suicidal ideation is frequently underreported and underrecognized in Primary Health Care, largely due to adolescents’ difficulties in verbalizing their suffering and professionals’ challenges in identifying early warning signs, particularly among male adolescents. The analyzed studies highlight structural, operational, and educational weaknesses within the health care network, while also revealing important potentialities, such as the use of soft technologies, qualified listening, bond building, intersectoral actions, and shared care planning. It is concluded that nursing plays a key role in suicide prevention by acting in a sensitive, welcoming, and politically committed manner toward the promotion of life. To support this role, continuous investments in professional training, work structure, and the strengthening of public mental health policies are necessary.
