Abstract: Polycystic ovary syndrome (PCOS) is one of the most prevalent metabolic disorders in women of childbearing age. Preeclampsia, is characterized by the onset of high blood pressure in pregnancy after 20 weeks, in the absence of previous hypertension, and is one of the main causes of adverse obstetric and perinatal outcomes. The aim of this study was to evaluate whether there is a relationship between PCOS and preeclampsia. A search for previous works was conducted in the National Library of Medicine (PubMed) and the National Health Library-Brazil (BVS-Brasil) databases, using the descriptors "polycystic ovary syndrome" and "preeclampsia". Articles published in the last 10 years (2015-2025) were included, provided they were controlled clinical trials and observational studies in English. Articles that were not within the scope of the study and paid articles were excluded. A total of 32 scientific articles were included after applying the inclusion and exclusion criteria. Most of the analyzed studies demonstrated an association between PCOS and preeclampsia, with hyperandrogenism and overweight being the main risk factors for the development of this complication in pregnant women with PCOS. Based on this evidence, it is concluded that investigating the history of PCOS during prenatal care is an important strategy for predicting the risk of preeclampsia.
