Abstract: Cardiovascular diseases (CVD) are currently a leading cause of global mortality, particularly in low- and middle-income countries, influenced by both modifiable and non-modifiable risk factors. Brazil presents regional inequalities and gaps in healthcare access that have contributed to CVD being the primary cause of death since the 1960s. This study aimed to describe the mortality rate from cardiovascular diseases in the state of Maranhão between 2012 and 2022. This is a quantitative research with an ecological study method. Secondary data on CVD mortality were obtained from the Mortality Information System (SIM) via the DATASUS platform. The results showed a total of 89,057 deaths during the period. A predominance was observed in males (57.2%), in the age group of 60 to 79 years (44.3%), and among individuals of mixed-race (Pardo) ethnicity (68%). Regarding social determinants, individuals with no formal education (48%) and those with a married marital status (44.7%) stood out. Geographically, the São Luís macro-region accounted for 31.8% of cases, although 71% of occurrences took place outside the metropolitan region. Ischemic heart diseases were the main underlying cause of death (40.9%), occurring mostly in hospital settings (49.8%). It is concluded that CVD mortality in Maranhão presents high rates and a well-defined epidemiological profile. Improving preventive practices and health education policies is essential, with special attention to the elderly and socioeconomically vulnerable groups to reduce these indicators.
