Abstract: Ekbom syndrome, also known as delusional parasitosis, is characterized by a fixed belief of bodily infestation by parasites in the absence of clinical evidence. It is a rare condition, more commonly observed in adults and older individuals, and may lead to significant psychological distress, self-inflicted skin lesions, and functional impairment. In Primary Health Care, the recognition of this condition represents a diagnostic and therapeutic challenge. This chapter reports a clinical case managed at a Primary Health Care Unit in a small town in the Brazilian Amazon, highlighting the importance of early recognition, an empathetic approach, and appropriate pharmacological management in socially vulnerable settings. Treatment with low-dose risperidone proved effective, leading to significant symptom remission within a short period.
