Abstract: Venous leg ulcers represent the most severe manifestation of Chronic Venous Disease and constitute a significant public health concern due to their high prevalence, chronicity, recurrence rates, and socioeconomic burden. These lesions result from sustained venous hypertension associated with microcirculatory changes, chronic inflammation, and dysfunction of the calf muscle pump, factors that impair the wound healing process. This study aimed to analyze current therapeutic approaches and emerging perspectives in the treatment of venous leg ulcers, based on recent scientific evidence. A narrative literature review was conducted using the databases LILACS, MEDLINE, SciELO, and PubMed, including publications from 2019 to 2025 in Portuguese and English. Findings indicate that compression therapy remains the gold standard treatment, as it directly addresses venous hypertension, reducing healing time and recurrence rates. Conventional dressings and therapeutic exercise play important adjunctive roles in clinical management. Among advanced therapies, neuromuscular electrical stimulation, biological and regenerative therapies, and technological approaches such as negative pressure therapy and photobiomodulation show promising results, particularly in refractory cases. It is concluded that venous ulcer management should be multimodal, individualized, and evidence-based, integrating conventional therapies and, when appropriate, advanced approaches to optimize healing outcomes and reduce recurrence.
