Abstract: This study investigated perceived changes and common factors of therapeutic effectiveness in existential psychotherapy from the patients’ perspective. A cross-sectional quantitative design was conducted with 57 adults receiving private clinical care in Brazil. Participants completed the Perceived Change Scale (PCS) and the Existentialist Psychotherapy Common Factors Scale, assessing psychological, relational, and behavioral dimensions, as well as therapeutic alliance and patient co-responsibility. Results indicated significant improvements in personal problems, coping with difficult situations, emotional stability, and interpersonal relationships. Less expressive changes were observed in physical health, sleep, appetite, and sexual satisfaction. The therapeutic alliance was rated very positively, particularly regarding mutual respect and the sense of acceptance. Patient co-responsibility and engagement were strongly associated with perceived outcomes. A positive, although weak, correlation was found between perceived family support and the sense of regaining control over one’s life. Findings suggest that existentialist psychotherapy promotes meaningful psychosocial changes supported by relational and contextual factors, contributing to the empirical evaluation of this clinical approach.
